In-Office Benefits
To better assist patients who do not currently have dental insurance, we offer the following In-Office Benefits Program

Our in-office benefits program provides comprehensive and professional dental care to our patients without claim forms, pre-authorizations or deductibles. Patients pay a yearly fee and receive the following services at no additional cost:

• All X-rays taken during initial Exam
  
• Comprehensive Exam
  
• Periodic Eam
  
• Adult Cleaning
  
• Adult Fluoride Treatment
  
• Child Cleaning
  
• Child Fluoride Treatment
  

The following services will be discounted at 50% off the cutomary fees as long as they are completed within the same year:

• 6 Month – Periodic Exam
  
• 6 Month – Adult Cleaning (2nd cleaning of the year)
  
• 6 Month – Adult Fluoride Treatment
  
• 6 Month – Child Cleaning and Fluoride Treatment
  
• 6 month -X-rays taken during the  6 month Exam
• All Dental Sealants

* ALL OTHER PROCEDURES WILL BE DISCOUNTED AT 15% OFF THE CUSTOMARY FEES.
